I use a subscription-based app called Sheet Music Scanner, though others prefer Piascore, Playscore 2, KeyScore or others.

It takes a little prep, though. Each score I scan has been cut down the fold to produce pages that load into the scanner tray. Press a button, and voila! Definitely worth it if you have more than a few dozen scores, IMHO. ... ForScore is a fantastic resource for cataloging music.

$30 a year or $6 a month for any music: multi page scores and PDF scores. Recognised as very accurate with full notation, even tremolos. Depending on the sheet music the results can be anywhere from good to horrible. The software that Musescore.com used last I checked is Audiveris and running it interactively can provide better results that the fully automated process.
